Construction Gate Code Request Form - Please use this form to formally request a construction gate code specifically for large construction projects. The code you request will require you to provide an End Date, which indicates when the project is expected to be completed. It is important to note that this code IS NOT to be shared with any individuals who are not associated with the project, as sharing it improperly will result in the code being revoked.

Medical Access Code Request Form - Use this form to request a code specifically for medical professionals to gain access to the gate and visit your home for a temporary period of time. Examples of individuals who may need this access include visiting nurses, hospice care workers, and other healthcare providers. It is important to provide an end date for this access, but please note that this date can be extended if necessary, in order for the form to be properly processed.

Party Code Gate Request Form - Use this convenient form to request a temporary, one-day access code for your guests to open the gate. Please note that, due to the security measures in place for the lake, the access code will be deleted without prior notice immediately after the day of your event.

Service/Utility GATE CODE Request Form - Please use this form to request a unique CODE specifically for utility vendors or service companies that provide essential services.

OTHER FORMS

Building Permit Application - Required for the construction of new homes, garages, additions, porches, patios, fences, and various other residential improvements, permits are an essential part of the building process. These necessary approvals ensure that all construction work meets local safety and zoning standards.

Boat Registration Form - All boats that are owned by lot owners at Lake Wauwanoka must be properly registered with the Lake authorities and are required to display a valid Lake sticker. This ensures that all watercraft are accounted for and adhere to our community guidelines.

Dock Checklist Form - All dock construction endeavors necessitate obtaining the appropriate approvals first from the Building Committee. It is essential to download and thoroughly review the Dock Checklist to ensure that everything is in proper order before proceeding with any plans.

Dock Construction Permit - Please use this form when seeking approval to construct a dock at our location.

Dock Transfer Form - Please use this form to update the dock assignment as a result of the recent sale of a home. It is important to ensure that the assignment reflects the changes stemming from this transaction.

Fence Permit Form - Please submit this form to initiate the process of installing a new fence, and be sure to include a copy of your property survey along with it for reference.

First Right of Refusal - This form is specifically utilized to formally notify the adjoining neighbors of a pending sale contract on a property, as well as to inform them of their option to purchase the property prior to the closing process with the new Buyer.

Security Access Form - For new or replacement gate key cards or remotes.

Service/Utility GATE CARD Request Form - Use this form to formally request a gate CARD for utility vendors or service companies that require access to the premises. This access is essential for carrying out their necessary services and maintenance work effectively.

Stock Waiting List Form (Additional Purchase) - Submit this form to ensure that your name is officially added to the waitlist for the opportunity to purchase additional shares of Lake Wauwanoka Stock.

Stock Waiting List Form (Initial Purchase) - Submit this form in order to request your INITIAL share of Lake Wauwanoka stock and ensure that your name is added to the waiting list. Please note that first-time purchasers are limited to acquiring only one share of stock. Owning your share of stock gives you the opportunity to participate in the yearly stockholder meeting that takes place in October, where you will also be able to cast your vote with your initial share. * For all subsequent requests, please use the Stock Waiting List Form.

Tri-Party Lease Agreement - If you are a landlord at Lake Wauwanoka and you rent or lease your house, please submit this form to the Board for processing.

Vehicle Sticker Request Form - If you choose to park it here, you need to ensure that you have one parking pass for each vehicle, including motorcycles, golf carts, and ATVs. This policy helps to maintain order and ensure that all vehicles have been registered with Lake Wauwanoka.
